Randomized Controlled Trials: Questions, Answers and Musings
There is a year difference in age between us and we come from vastly different backgrounds. Nevertheless, during more than a decade we have recognized the extent to which our paths and thinking have converged. We first noticed this in Oxford with our mutual interest in systematic reviews and confirmed it at McMaster University as we became painfully aware of the pitfalls of over reliance on quantitative evidence and its limited influence on health care. During the past 5 years, our focus has been more on the value of rhetoric, information and communication technologies, complex systems and the importance of effective communication in the health system.
Over the years, we have grown together in our understanding, have worked together on a variety of projects, and mutually reinforced our views on clinical evidence. We are happy to share our expanding vision of randomized trials in this new edition of the book.

After 13 years there are new areas to discuss and more recent trials to be included. Good clinical practice; evaluation of quality of life; measurement of the benefit:risk comparison; determination of cost- effectiveness and cost utility; stopping rules for trials; meta-analysis and subgroup analysis are all new sections. The references are expanded from to and include the recent advances in trial design, such as the n-of-1 trials and megatrials, and up-to-date examples to illustrate the points made in the 20 chapters.

Randomiserad kontrollerad studie
Randomiserad kontrollerad studie (förkortat RCT efter engelskansrandomized controlled trial, även randomiserad interventionsstudie) är en bland annat inom klinisk medicinsk forskning använd studiedesign där deltagarna slumpmässigt, det vill säga randomiserat, väljs till den grupp som får den intervention/behandling som ska studeras eller till en kontrollgrupp.
Användningen av en kontrollgrupp vid sidan av den grupp som får till exempel en ny form av medicinsk behandling, är viktig för att kunna påvisa effekten av just den studerade interventionen, den givna behandlingen. Vid en klinisk prövning är det också viktigt att jämföra effekten mot gängse medicinsk behandling. Randomiseringen görs för att så långt som möjligt få ett representativt urval i både studie- och kontrollgrupp.[1]
Om möjligt, bör studien dessutom genomföras som dubbelblind studie, det vill säga att varken försöksledare, vid klinisk studie de som ger behandling och bedömer resultat, eller studiedeltagarna själva känner till vilken av de jämförda behandlingarna som ges i det enskilda fallet.
